Daily Duties

  • Review menu, sign diet orders, sign memos
  • Set up staff dining room
  • Refill all juices, coffee, napkins, dressing, creamers, oleo, butter, coffee rich, etc. 
  • Bag bread dining room
  • Set up dining room (bread, butter, straws, napkins, sugar bowls, creamers, crackers, coffee cups, empty glasses, etc., prepare all drinks - thickens and regulars

Nursing Department

Charge Nurse, R.N. & L.P.N.

General Description:

  • Reports to Director of Nursing Service
  • Supervises and directs the activities of assigned team members during the shift
  • Provides nursing care according to physician's orders and in conformance with recognized nursing techniques and procedures, established standards and procedures, established standards and policies
  • Maintains accurate and complete records of nursing observations and care
  • Recognizes and interprets symptoms, reporting resident's condition
  • Aware of residents' conditions at all times
  • Assists the physician in diagnostic and therapeutic measures
  • Performs other related duties as assigned

 Nurse Educator, R.N.

General Description:

L.P.N. for Adult Day Center

General Description:

  • Reports to the Director of Nursing/Director of the Adult Day Center
  • Must know and follow the rules and regulations pertaining to the ODA for the ADC
  • Informs R.N. about new clients and schedules the initial assessments of each consumer in a timely manner
  • Draft an activity plan for each consumer in a timely manner
  • Administers and maintains accurate and complete records of the Two Step Mantoux for each consumer
  • Performs the Interdisciplinary Care Conferences for each consumer in a timely manner
  • Must be aware of the consumer's condition and reports any changes to the consumer's physician caregiver and their case manager

State Tested Nursing Assistant

Tours of Duty:

6 a.m. to 2:30 p.m.

2 p.m. to 10:30 p.m.

10 p.m. to 6:30 p.m.

Job Summary:

Provides direct and indirect resident care activities under the direction of a R.N or L.P.N. Assists residents with activities of daily living, provides for personal care, comfort and assists in the maintenance of a safe and clean environment for an assigned group of residents.

Medical Records

Part time, Per diem

Responsibilities:

  • Collecting, mailing/faxing patient information to insurance companies
  • Organizing a discharged chart into chronological order, while making sure all necessary paperwork is signed and dated properly 
  • Filing discharged charts
  • Pulling charts requested by state for review
  • Collects and records immunizations upon admission
  • Ensures the annual H&P and annual Mantoux are complete
  • Files a CMPT report monthly
  • Reviews H&P, orders and medication sections of charts for new admissions
  • Enters all ICD10 codes for new admissions
  • File, box and categorize older charts for filing in the basement
  • Obtain consents from resident, family member or guardian pertaining to annual pneumonia and influenza vaccines
  • MDS training/co-signing
  • Accruing signatures from physicians and nurses on all necessary documentation
  • Performs all other duties as assigned

Would Nurse 

Job Requirements:

R.N with special training in would care. They oversee the would care program and acts as a liaison between the nursing staff, families, hospitals and physicians regarding wound care. The RN assesses the resident's response to treatments. They train and supervise nursing assistants. Participates in resident and family teaching. Maintains role as resident advocate with focus on the facility's mission.
