Visitation Scheduling

Click on the image to schedule a visit. UPDATE - 1/18/2021- We will be restarting indoor visitation on 1/21/21. COVID-19 testing is required prior to each visit. Residents are permitted one visit per week. We are testing staff members twice per week. If any positive cases are found, visits will stop for at least 14 days. Vaccine Update

UPDATE 12/15/2020: As you are aware, the FDA has now approved Emergency Use Authorization for a COVID-19 vaccine. This is welcomed news given the catastrophic amount of deaths that have occurred in nursing homes all throughout the county. It is generally accepted that nursing homes account for about 8% of all COVID-19 cases but account for approximately 40% of all deaths. We strongly urge you to have discussions with your loved one or other family members in advance of the notice that Heritage Manor, with the assistance of Walgreens, will begin vaccination. At this time, we do not know when this will occur and we do not know how much notice we will receive.

Long & Short Term Care

Heritage Manor is rated a five-star skilled nursing facility by the Centers for Medicare & Medicaid services. It provides an adult day services program and is the only Veterans Administration approved provider of long term care in Mahoning County. Heritage Manor offers 24-hour licensed nursing care seven days a week. All types of physical, occupational, and emotional therapy are offered seven days a week. Counseling services are available to residents and their families, and those who are undergoing physical therapy. Click the image to learn more.
