WE’RE REACHING OUT TO YOU
Come to us and we’ll help you navigate the mature stages of life! Our outreach coordinator and social workers welcome the chance to talk with seniors, helping them meet the needs that start to come up as you get older.
WHAT DOES THAT LOOK LIKE?
This free service includes a multitude of aspects, including educational awareness from our Aging Well programs, as well as home visits from our Senior Outreach Coordinator. These home visits are focused on you. We’ll take out all the stress that comes with navigating transitions, resources, and care. We’ll come to you with all the information and answers to questions you’ve been searching for.
Our Senior Outreach Coordinator is supported by the generosity of the Trumbull and Mahoning County Mental Health and Recovery Boards.
WE ENCOURAGE YOU TO BE ACTIVE, INDEPENDENT, AND CONNECTED
Our hope is that all of the seniors in our community will be active in our community and maintain their independence. And when you need extra care, we’ll help you explore your options. Sometimes that’s home health care, hanging out at an adult day services program, joining an assisted living community, healing at a rehabilitation center, or moving to a new home with care available around the clock.
Take a look at some of the programs and facilities we recommend to our community members! This Senior Living group is home at an adult day services program, assisted living facility, rehabilitation center, and a skilled nursing facility with an active community.
